When requesting a Non-durable Power of Attorney for Physical and Health Care of a Minor Child and the parent lives in one state and the agent in another, would you use the form that is state specific to where the parent lives?

We recommend you to use your state of residence Power of attorney, and write in it that this document is to be used in state of residence of your agent that will take care of your child.
