First of all, communicate your wishes to your immediate family and leave a copy of your Advance Heath Care Directive with a member of your family. Give the same to your designated agent. Before you execute an Advance Heath Care Directive talk to your doctor and make sure you understand the meaning of all medical procedures possible. Ask that copies be placed in your medical file, and make sure the hospital also has a copy of this important document.
