An advance medical directive is any written instruction concerning the making of medical treatment decisions
on behalf of the person who has provided the instructions.
|
|
|
|
|
You have the right to make decisions about your health care. No health care may be given to you over your objection, and necessary health care may not be withdrawn or withheld if you object.
The Alabama Advance Health Care Directive combines into one document (a) the Designation of Health Care Proxy, and (b) the Living Will.
However, Alabama Law does not require you to follow the formula "two documents in one." For example, you may choose to appoint your agent
to make important health-related decisions for you in a Designation of Health Care Proxy (Power of Attorney for Health Care) and not to sign
a Living Will.

consists of the following parts: Part 1 - a Living Will (instructions for health care) and Part 2 a Designation of Health Care Proxy.
