According to Florida law, an Advance Health Care Directive is a witnessed written document or oral statement directing
that life-sustaining procedures be withheld or withdrawn. It also defines other health care treatment decisions that can be made on your behalf.
|

In this statement, you (the “Principal”) give instructions or express your desires concerning any aspect of your health care. The statement
includes, but is not limited to, the designation of a health care surrogate, a living will, or an anatomical gift made pursuant to Florida Statutes.

consists of three parts: Part 1 - a Living Will including anatomical gift provisions; Part 2 - a Designation of Health Care Surrogate (agent);
