According to Illinois law, an adult may prepare a written Advance Health Care Directive.
If you do not have a Health Care Directive and you can not communicate your wishes to the doctor.

An advance medical directive is a written instruction concerning the making of medical treatment decisions on behalf of the person
who has provided the instructions. You have the right to make decisions about your health care. No health care may be given to you over
your objection, and necessary health care may not be stopped or withheld if you object.
In accordance with Illinois law (Code 755 ILCS 35) our website provides you with a choice of purchasing (a) Illinois Advance Health Care Directive with Mental Treatment Preference Declaration
and Organ Donation Provision, (b) Illinois Durable Power of Attorney for Health Care, and (c) Durable Springing Power of Attorney for Property and Finance forms
