Michigan Law allows for both a Durable Health Care Power of Attorney and a Living Will. The Health Care Power of Attorney and the Living Will are called Advance Directives.
No health care may be given to you over your objection, and necessary health care may not be stopped or withheld if you object.
Our website provides you with a choice of purchasing Michigan Advance Health Care Directive which includes (a) Michigan Health Care Directive to Physician (Living Will),
(b) Michigan Power of Attorney for Health Care and (c) Michigan Springing Durable Power of Attorney Property
and Finances Upon Disability forms in one package; or, you may purchase one (or all) of these legal documents separately:

|
- Health Care Directive to Physician (Living Will) with Organ Donation Provision
- Durable Power of Attorney for Health Care with Organ Donation Provision
