According to Oregon law, an Advance Health Care Directive helps you to communicate your wishes about medical treatment for an event when you are unable to make health care decisions
According to Oregon law, an adult may prepare a written statement known as an Advance Health Care Directive that controls health care treatment decisions at the time when you are unable to communicate your wishes; and allows life-sustaining procedures be withheld or withdrawn.
No health care may be given to you over your objection, and necessary health care may not be withdrawn or withheld if you object.
In this statement, you (the “Principal”), give instructions or express your desires concerning any aspect of your health care.
Our website provides you with a choice. You may purchase a single document called "Oregon Advance Health Care Directive," which consists of the following parts: an Appointment of Health Care Representative (Agent); a Health Care Instructions (Living Will); and a Durable Springing Power of Attorney for Property and Finance. OR, you may purchase and download the following documents separately:
- Power of Attorney for Health Care (equivalent of an Appointment of Health Care Representative)
- Oregon Declaration for Mental Health Treatment
- Springing Durable Power of Attorney for Property and Finance
