If you do not have a Health Care Directive and you become incapacitated or incompetent you can not communicate your wishes to the doctor.
According to Delaware Statutes, any competent adult may execute an Advance Health Care Directive.
|
|
|
|
|
Delaware Advance Health Care Directive consists of four parts and combines into one document a Health Care Power of Attorney, Living Will (Instructions for Health Care), Guardianship and Organ Donation provisions.
It also allows you to designate a Primary Physician.
However, you are not required to complete all parts of this Advance Directive form. You may chose to appoint your health care agent to make important health-related decisions for you, or to leave directions on kinds of life-saving treatment you want (do not want) in case you are terminally ill.
