According to Washington Statute, any competent adult may execute a document directing that
life-sustaining procedures be withheld or withdrawn. It also defines other health care treatment decisions that can be made on your behalf.

You have the right to make decisions about your health care. No health care may be given to you over your objection, and
necessary health care may not be stopped or withheld if you object.

(b) Washington Durable Power of Attorney for Health Care, and (c) Washington Durable Springing Power of Attorney for Property and Finance forms
