The structure of the Oklahoma Advance Health Care Directive is regulated by the "Oklahoma Rights of the Terminally Ill or Persistently Unconscious Act".
It defines health care treatment decisions that can be made on an adult person’s behalf.

|
In Oklahoma, an individual of sound mind and eighteen (18) years of age or older may execute an advance directive governing the withholding or withdrawal of life-sustaining treatment.
The Oklahoma Advance Health Care Directive combines into one document a Living Will, a Power of Attorney for Health Care,
and a Directive regarding organ donation. However, you are not required to complete all parts of the Oklahoma Advance Directive form.
