According to Texas law, an adult may prepare a written Directive to control the health care treatment decisions
that can be made on that person's behalf

Our website provides you with the following choices with regards to Texas Advance Health Care Directive:
1. You may purchase a package of forms consisting of

- Texas Advance Health Care Directive (Living Will or Directive to Physicians and Family or Surrogates);
- Texas Medical Power of Attorney;
- Texas Durable Springing Power of Attorney for Property and Finance.

2. OR, you may purchase some or all of these forms separately.

|
|
"Medical power of attorney" (in other words "Power of Attorney for Health Care") is a document delegating to an agent authority to make health care decisions for you.
"Health Care Directive to Physicians and Family or Surrogates" (equivalent of Living Will) is designed to help you communicate your wishes now about medical treatment at some time in the future when you are unable to make your wishes known because of illness or injury. You may state whether you want/do not want CPR, tube nutrition or hydration, whether you want an autopsy.
"Springing Durable Power of Attorney upon Disability or Incompetence" gives the person whom you designate (your "agent", "attorney-in-fact") broad powers to handle your property, tangible or intangible.
