According to Utah Statute: Health Care Directive helps you to communicate your wishes about medical treatment for an event when you are unable to make health care decisions
According to Utah law, a person 18 years of age or older may execute a Health Care Directive. The Directive is binding upon
attending physicians and all other providers of medical services.
An advance medical directive is any written instructions concerning the making of medical treatment decisions on behalf of the
person who has provided the instructions. You have the right to make decisions about your health care.
No health care may be given to you over your objection, and necessary health care may not be withdrawn or withheld if you
object.
According to Utah law the directive shall be in substantially statutory form or in a form substantially similar to the form approved by prior Utah law.
Our website provides you with a choice of purchasing one package containing the forms approved by Utah law (a) Utah Directive to Physicians and Providers of Medical Services (Living Will), (b) Utah Statutory Special Health Care Power of Attorney, and (c) Durable Springing Power of Attorney for Property and Finance; or, you may purchase any of these forms separately:
- Utah Directive to Physicians and Providers of Medical Services (equivalent of Living Will)
- Utah Statutory Special Health Care Power of Attorney
- Utah Declaration for Mental Health Treatment
- Durable Springing Power of Attorney for Property and Finance
